How do you report a bad umpire to ASA?

This weekend we played in a softball tourney and we had the same umpire the WHOLE time and he HATED us with a passion he also broke a few ASA rules (1 of our coach has been an ASA ump. 4 bout 20 years) and I don't think other teams should be treated this way so how do I report him?
