Why do my New Vegas pre-order package not appear on my new game?

I just started a new game on New Vegas the other day and when I pre-ordered it long ago, I got an add-on. I've played and beaten the game and now I'm going again. I'm just wondering if anyone knows why when I started my game, the pre-order add-on didn't appear? I want my grenade rifle!
